Admission Guidelines for Qualified Students

What To Do

Qualified students must prepare two (2) complete sets of requirements (1 original and 1 photocopy). Documents must be uploaded in the IAIMS and submitted as hard copies during the admission schedule.

For Transferee Students

• Certificate of Eligibility to Transfer / Honorable Dismissal
• Transcript of Records with remarks (for evaluation/reference)
• NSO/PSA Birth Certificate (original or clear copy, per RA 11909)
• Clear copy of Certificate of Live Birth (if NSO/PSA copy is unreadable)
• Two (2) recent 2×2 ID pictures (white background preferred)
• Self-Certification Form

For New Students

• Report Card / School Form 9
• Certificate of Good Moral Character
• NSO/PSA Birth Certificate (original or clear copy, per RA 11909)
• Clear copy of Certificate of Live Birth (if NSO/PSA copy is unreadable)
• Two (2) recent 2×2 ID pictures (white background preferred)
• Self-Certification Form

Medical Clearance (Submit to College Clinic)

• Medical Certificate (preferably from a government physician)
• Drug Test Result

Optional Requirements (If Applicable)

• Marriage Certificate (for married women students)
• Letter-request for change of civil status
• Clear copy of Certificate of Live Birth (if NSO/PSA copy is unreadable)
