STEPS ON HOW TO REQUEST FOR CREDENTIALS VIA GOOGLE FORM

Step 1: Access this link for the request for credentials (RFC).

Step 2: Fill up all the fields being required of. Review first before submitting.
Step 3: Wait for the reply. kindly read further instructions and comply the following in person:

  1. a. Requirements in original copies not yet submitted during the enrollment period, sf 10/OTR for DNSC. (please disregard if submitted)
  2. b. If the request needs submission of specific documents, refer the checklist of requirements.
  3. c. Accomplished/approved student clearance form. (for transfer, employment, graduation credentials, and, for specific legal purpose, one time only)
  4. d. Request payment slip with official receipt.
  5. e. Documentary stamp/s, one doc stamp for every document. (per sec.188, National Internal Revenue code of 1997 & CHED order no. 30 s. 2010)

Step 4: Pay online or over the counter. To pay online via landbank link.biz portal, please provide the correct student name enrolled/used at DNSC. A student number will be asked. access the LBP link.biz portal thru this link:

  1. Go to the landbank website and click on the landbank link.bizportal.
  2. On the link.bizportal site, select the “pay now” to begin.
  3. Choose merchant from the list or enter the Davao del Norte State College in the search bar. Then, click “continue”.
  4. Select the transaction type and click “continue”.
  5. Input the required account information and transaction details. Choose service fee registrar payment details – type of fee: document
  6. Select preferred mode of payment, enter the 10-digit lanbank account number. For clients who will use other payment options, will be redirected to the respective payment gateway page of your choice for the processing of the transaction. Proceed to the “captcha” challenge. Read the terms and conditions before checking the box provided. Click “continue” to the next page.
  7. Make a final review the details you’ve provided before inputting the OTP (one-time -password), JAI and the atm card pin using the pin pad on the screen. Then, click “submit”. A “payment confirmation receipt” will be displayed upon successful transaction.

Step 5: If payment online is made:

  1. a. Email the proof of payment or the “payment confirmation receipt” at finance@dnsc.edu.ph add cc registrar@dnsc.edu.ph
  2. b. The proof of payment will be checked by the finance office for confirmation and for the issuance of official receipt (OR).
  3. c. Wait for the confirmation from the finance services office.
  4. d. The registrar’s office will wait for the official receipt (OR) from the cashier’s office.
    step 6: If payment is made over the counter or online, submit the RFC form, student clearance, request payment slips with the official receipt, the documentary stamp/s, and other specific documents at the registrar’s office.

Step 7: A request slip-student’s copy with schedule/claim stub to claim the request will be provided.(incomplete requirements, no schedule for release yet.) Email registrar@dnsc.edu.ph or call 0963-128-0320 for updates or clarifications.

Step 8: Access this link for the online appointment and present the confirmation to the guard on duty or bring any valid id.

Step 9: Present the request slip-student’s copy/claim stub in claiming the request. Important: if the requested document/s are to be requested & to be claimed by his/her representative, the authorized person shall submit:
a) An authorization letter by the student duly notarized or a notarized special power of attorney (spa) or an affidavit of undertaking, or a consularized special power of attorney at the place of the student’s
residence abroad with photocopy of a valid passport
b) clear copy of any valid id of the student
c) clear copy of any valid id of the authorized person

Registrar’s office shall try to weigh the documents using the document weighing scale
(example only) for this transaction (1 document), this is considered as JRS’:
a.  One pounder, Mindanao, minimum of 500 grams, worth P130.00.
b.  in excess of 500 grams or a fraction thereof, add  P65.00 =P195.00

  1. The student shall print or e-mail the proof of payment to the dnsc registrar’s office,  registrar@dnsc.edu.ph
  1. The registrar staff in-charge will then inform the JRS Panabo regarding the document to be mailed and to send the proof of payment for confirmation.
  2. The courier or the JRS Panabo will pick up the document to be mailed on the date of release of the requested document.
  3. The courier or the JRS Panabo will sign the DNSC mailing log book and will issue the official receipt.

DELIVERY VIA COURIER USING DHL W EXPRESS (INTERNATIONAL OR LOCAL)
